read the directions below carefully. Not every requirement applies to every
major. Please only complete those requirements that apply to you!
It is your responsibility to remember and meet the due dates!

1. Student
Information - Any major currently taking an
education course must complete the
following.
a. By the
deadline given above complete the Student Information form in
Task Stream. Make certain you complete the form in the course for which you
are enrolled. (If you are not taking an education
course, do not complete this form.)
b. You will be asked for the following information. If your form is incomplete, your
professor will have to contact you for the additional information before you
can be assigned to a mentoring partner or to a campus. A delay may cause you to miss the deadline
dates.
1. complete
name - If you do not have a middle name, please type NMN so we'll
know you didn't just forget your middle name.
2. any other last names (for example, maiden name) - If you do not
have any other last names, please type none so we'll know you
didn't just forget to answer the question.
3. current phone number
4. current email address
5. social security number (or student ID number if you have no SSN
... please identify it as a student ID number)
6. FALL - give birth date in MM/DD/YYYY format and complete
remaining questions (All majors need a new ID every fall.)
SPRING/SUMMER - If you have a current Education
Department ID, type have current ID & do not
complete remaining questions.
SPRING/SUMMER - If you do not have a current ID & are
enrolled in TECA 1354, type TECA 1354 & do
not complete the remaining questions.
SPRING/SUMMER - If you do not have a current ID & will
be completing an internship give birth date in MM/DD/YYYY format
and complete the remaining questions.
SPRING/SUMMER - If you lost your current ID, type lost
ID & complete next question only.
7. current local mailing address to which we will mail your department
ID
8. race/ethnicity

c. Complete the
Consent & Release form ONLY if you do not have a
current department ID & have not already completed a
C&R form since the last August. (If you have a current ID or lost your ID,
do not send another C&R form.) Print the Consent & Release form (on the Consent & Release page linked
above), sign & date it, and give it to your professor by the deadline
given above. Please check your course syllabus for instructions on how to
get your C&R form to your professor. d. If you do not receive your
department ID card in the mail by
the deadline given above, you
should immediately let your professor know.
If you miss any deadlines, you may be unable to complete your class/semester requirements so don't be late!
2. Campus Choice
for online & on campus TJC students
a. A list of
participating campuses is posted on the Campuses page by the deadline given
above.
b. You must submit the Campus Choice form in Task
Stream giving your three campus choices by the
deadline given above.
(Please see the instructions on the Campuses page.) Majors currently
enrolled in TECA 1318 & EDUC 2301 are given first pick of assignments.
Majors enrolled in other courses are assigned on a first come, first served
basis. You are always given your first choice if it is available, then your
second, and so on. Do not submit your form until after the
participating
campuses are posted.
c. Intern lists are sent to the schools and campus assignments are posted
on the Campuses page by the deadline given above. If you miss
the deadline to give your choices to your professor, you may not get
a school! Do NOT contact any school until the campus assignments are posted.
d. If you registered for your class at a college other than TJC,
ignore steps a-c above and see Campus Choice for VCT & Distance Students
below.
e. If you registered for your class through TJC but you live more
than one hour's drive from Tyler, ignore steps a-c above and see Campus
Choice for VCT & Distance Students below.
3. Campus Choice for VCT &
Distance
Students
(Step 3 ONLY applies to
certain students. If 2d or 2e above do not apply to you, you should NOT
complete this step.)
a. Locate a public school in your area
at which you would like to complete your internship. You may make a
general request of the principal. Tell the principal that your department will contact
him/her to explain the internship and see if the school is interested in hosting
any interns.
b. By the deadline given above, submit the Campus Choices
form in Task Stream giving the
following information.
1. your complete
name 2. the college through which you registered for this course (if VCT)
or the town in which you are currently residing (if you registered
through TJC, but live more than one hour's drive from Tyler) 3. the
local public school at which you wish to complete your internship 4. the principal's name, phone number, and email address
c. We will
contact you as soon as we hear from the school you requested. Keep in mind,
though, that we have little choice but to wait on them. d. Once
you've received a confirmation from your professor follow all other
directions on this page and on the linked pages.
