CONSENT AND RELEASE

            I, _______________________, hereby give my consent and authorization to Tyler Junior College (“TJC”), its employees, faculty, agents, or third parties hired by TJC to conduct a background check and/or investigative consumer report on me.  I understand that this report may be conducted because I am entering into a public school as a representative of TJC.

            I agree to hold TJC, its employees, faculty, agents, or third parties hired by TJC harmless as to any act, omission, or information having to do with the investigation or preparation of this report, and hereby release TJC from any and all liability arising from or relating in any way to  the conduction of a background check and/or investigative consumer report.

Tyler Junior College (“TJC”) and its employees, faculty, agents, or third parties hired by TJC,  may conduct a background check and/or investigative consumer report on paid and unpaid representatives of the college that are present in public schools.

This report may include information related to your credit, employment, or criminal history, in addition to other matters.

If you consent to TJC conducting this background check, then please sign the attached Consent and Release form.
